    Distribution of the brown bear (Ursus arctos marsicanus) in the Central Apennines, Italy, 2005-2014

    Despite its critical conservation status, no formal estimate of the Apennine brown bear (Ursus arctos marsicanus) distribution has ever been attempted, nor a coordinated effort to compile and verify all recent occurrences has ever been ensured. We used 48331 verified bear location data collected by qualified personnel from 20052014 in the central Apennines, Italy, to estimate the current distribution of Apennine brown bears. Data sources included telemetry relocations, scats and DNA-verified hair samples, sightings, indirect signs of presence, photos from camera traps, and damage to properties. Using a grid-based zonal analysis to transform raw data density, we applied ordinary kriging and estimated a 4923 km2 main bear distribution, encompassing the historical stronghold of the bear population, and including a smaller (1460 km2) area of stable occupancy of reproducing female bears. National and Regional Parks cover 38.8% of the main bear distribution, plus an additional 19.5% encompassed by the Natura 2000 network alone. Despite some methodological and sampling problems related to spatial and temporal variation in sampling effort at the landscape scale, our approach provides an approximation of the current bear distribution that is suited to frequently update the distribution map. Future monitoring of this bear population would benefit from estimating detectability across a range on environmental and sampling variables, and from intensifying the collection of bear presence data in the peripheral portions of the distribution

    Strong association between non alcoholic fatty liver disease (NAFLD) and low 25(OH) vitamin D levels in an adult population with normal serum liver enzymes

    <p>Abstract</p> <p>Background</p> <p>Hypovitaminosis D has been recently recognized as a worldwide epidemic. Since vitamin D exerts significant metabolic activities, comprising free fatty acids (FFA) flux regulation from the periphery to the liver, its deficiency may promote fat deposition into the hepatocytes. Aim of our study was to test the hypothesis of a direct association between hypovitaminosis D and the presence of NAFLD in subjects with various degree of insulin-resistance and related metabolic disorders.</p> <p>Methods</p> <p>We studied 262 consecutive subjects referred to the Diabetes and Metabolic Diseases clinics for metabolic evaluation. NAFLD (non-alcoholic fatty liver disease) was diagnosed by upper abdomen ultrasonography, metabolic syndrome was identified according to the Third Report of National Cholesterol Education Program/Adult Treatment Panel (NCEP/ATPIII) modified criteria. Insulin-resistance was evaluated by means of HOMA-IR. Fatty-Liver-Index, a recently identified correlate of NAFLD, was also estimated. Serum 25(OH)vitamin D was measured by colorimetric method.</p> <p>Results</p> <p>Patients with NAFLD (n = 162,61.8%) had reduced serum 25(OH) vitamin D levels compared to subjects without NAFLD (14.8 ± 9.2 vs 20.5 ± 9.7 ng/ml, p < 0.001, OR 0.95, IC 95% 0.92-0.98). The relationship between NAFLD and reduced 25(OH)vitamin D levels was independent from age, sex, triglycerides, high density lipoproteins (HDL) and glycaemia (p < 0.005) and Fatty Liver Index inversely correlated with low 25(OH) vitamin D regardless sex, age and HOMA-IR (p < 0.007).</p> <p>Conclusions</p> <p>Low 25(OH)vitamin D levels are associated with the presence of NAFLD independently from metabolic syndrome, diabetes and insulin-resistance profile.</p

    Laparoscopic Pancreatoduodenectomy

    In recent years, total laparoscopic pancreaticoduodenectomy (TLPD) has been introduced as a feasible alternative to open pancreaticoduodenectomy (OPD) when performed by experienced surgeons in laparoscopic and pancreatic surgery. Its application has been gradually increased, but its safety, reproducibility, and oncological outcomes are still debated due to its technical complexity and prolonged operating time. We performed a systematic analysis of the more relevant aspects of TLPD. In this chapter, we report a general overview of the different experiences present in the literature regarding indications, surgical techniques, postoperative outcomes, benefits and limitations of this approach, oncological results, learning curve, and costs. There is no standardized surgical technique for TLPD. Different techniques exist for both the demolitive stage and the reconstructive stage. We summarized the different aspects of the surgical technique based on the various experiences reported by different authors. Compared to OPD, TLPD provides the advantages of laparoscopy, i.e., reduced blood loss, decreased postoperative pain, and shorter length of hospital stay, without increasing the rate of postoperative complications or compromising oncological outcomes. An appropriate patient selection is crucial at the beginning of the learning curve. With increased experience, more challenging cases may also be approached with this technique, including those requiring major vascular resections or multi-visceral resections

    Self-administration of omalizumab: why not? A literature review and expert opinion

    Introduction: Omalizumab is used to treat severe uncontrolled allergic asthma and chronic spontaneous urticaria (CSU), and is approved for self-administration in prefilled syringes. It is thus important to understand the advantages, critical issues, and indications for home administration.Areas covered: The present review summarizes the available evidence on home administration of omalizumab in asthma and CSU to illustrate the advantages derived from self-administration of patients in this setting.Expert opinion: The available data suggest that patients can safely administer biologics at home with suitable training, and that home administration is time saving and cost-effective. The majority of patients with severe asthma or CSU treated with omalizumab are likely to be suitable candidates for self-administration, which can be proposed to anyone that the clinician deems suitable. In addition to clinicians, pharmacists can also play a key role in managing patients who are prescribed home administration. A practical flow chart is proposed on selection of patients and their management during home administration. Self-administration of biologics can be considered as a valid alternative to traditional injections in a clinical setting, and the evidence has shown that no major issues need to be overcome in terms of safety or efficacy

    Solidification microstructure of centrifugally cast Inconel 625

    Centrifugal casting is a foundry process allowing the production of near net-shaped axially symmetrical components. The present study focuses on the microstructural characterization of centrifugally cast alloys featuring different chemical compositions for the construction of spheres applied in valves made of alloy IN625 for operation at high pressure. Control of the solidification microstructure is needed to assure the reliability of the castings. Actually, a Ni-base superalloy such as this one should have an outstanding combination of mechanical properties, high temperature stability and corrosion resistance. Alloys such as IN625 are characterised by a large amount of alloying elements and a wide solidification range, so they can be affected by micro-porosity defects, related to the shrinkage difference between the matrix and the secondary reinforcing phases (Nb-rich carbides and Laves phase). In this study, the microstructure characterization was performed as a function of the applied heat treatments and it was coupled with a calorimetric analysis in order to understand the mechanism ruling the formation of micro-porosities that can assure alloy soundness. The obtained results show that the presence of micro-porosities is governed by morphology and by the size of the secondary phases, and the presence of the observed secondary phases is detrimental to corrosion resistance

    Global diversity in the TAS2R38 bitter taste receptor: Revisiting a classic evolutionary PROPosal

    The ability to taste phenylthiocarbamide (PTC) and 6-n-propylthiouracil (PROP) is a polymorphic trait mediated by the TAS2R38 bitter taste receptor gene. It has long been hypothesized that global genetic diversity at this locus evolved under pervasive pressures from balancing natural selection. However, recent high-resolution population genetic studies of TAS2Rs suggest that demographic events have played a critical role in the evolution of these genes. We here utilized the largest TAS2R38 database yet analyzed, consisting of 5,589 individuals from 105 populations, to examine natural selection, haplotype frequencies and linkage disequilibrium to estimate the effects of both selection and demography on contemporary patterns of variation at this locus. We found signs of an ancient balancing selection acting on this gene but no post Out-Of-Africa departures from neutrality, implying that the current observed patterns of variation can be predominantly explained by demographic, rather than selective events. In addition, we found signatures of ancient selective forces acting on different African TAS2R38 haplotypes. Collectively our results provide evidence for a relaxation of recent selective forces acting on this gene and a revised hypothesis for the origins of the present-day worldwide distribution of TAS2R38 haplotypes

    A modular phage vector platform for targeted photodynamic therapy of Gram-negative bacterial pathogens

    : Growing antibiotic resistance has encouraged the revival of phage-inspired antimicrobial approaches. On the other hand, photodynamic therapy (PDT) is considered a very promising research domain for the protection against infectious diseases. Yet, very few efforts have been made to combine the advantages of both approaches in a modular, retargetable platform. Here, we foster the M13 bacteriophage as a multifunctional scaffold, enabling the selective photodynamic killing of bacteria. We took advantage of the well-defined molecular biology of M13 to functionalize its capsid with hundreds of photo-activable Rose Bengal sensitizers and contemporarily target this light-triggerable nanobot to specific bacterial species by phage display of peptide targeting moieties fused to the minor coat protein pIII of the phage. Upon light irradiation of the specimen, the targeted killing of diverse Gram(-) pathogens occurred at subnanomolar concentrations of the phage vector. Our findings contribute to the development of antimicrobials based on targeted and triggerable phage-based nanobiotherapeutics

    Accesso ai corsi di laurea in medicina: messa a punto di un questionario conoscitivo relativo a equilibrio personale e relazionale, attitudini, motivazioni e valori in contesti educativi (EMAV-E)

    La grande domanda di accesso ai corsi di medicina richiede di individuare nei candidati le caratteristiche che ne possano fare un “buon medico”. A tal fine abbiamo sviluppato un questionario (EMAV-E) conoscitivo su alcune caratteristiche dei candidati, come base di partenza per l’incontro con l’esperto selezionatore. Le macro-aree indagate dal questionario sono state: equilibrio personale e relazionale (equilibrio e autoregolazione), attitudini accademiche e professionali (intelligenza emotiva, metacognizione, motivazioni), valori personali e professionali, e desiderabilità sociale dell’individuo, usata come controllo. Le macro aree erano divise in 20 aree di interesse entro le quali sono stati individuati 238 item ritenuti rappresentativi. Per la validazione del questionario, sono stati reclutati 653 studenti iscritti al primo anno di corsi di laurea di area medica. Dal questionario iniziale sono state ricavate tre versioni brevi di 80 items. Per saggiarne l’effettiva confrontabilità sono state risomministrate a 160 studenti diversi dal campione iniziale. I tre questionari avendo parametri statistici paragonabili, possono essere considerati forme parallele dello stesso questionario. La procedura ha consentito di sviluppare uno strumento per l’individuazione di punti di forza e di debolezza, secondo le macro aree prese in considerazione, degli studenti che ambiscono a intraprendere la formazione universitaria presso i corsi di laurea in medicina o delle professioni sanitarie
